THE NCCED PRESCRIPTION BENEFIT CARD (PBC) PROGRAM
HOW TO OBTAIN A PRESCRIPTION BENEFIT CARD

TO REQUEST A PRESCRIPTION BENEFIT CARD: Simply submit to NCCED, the name, city and state, and gender of each person that is to be issued an NCCED/PBC. Date of birth is helpful - especially for more common names - but is not required. Also include the name, organization and phone number of the person requesting the card(s). For your convenience, a spreadsheet with space for the pertinent information is available on our website at www.ncced.org.
You can also request the card online.

NOTE: There is NO requirement to sign an application, provide a social security number, driver's license or photo ID in order to receive an NCCED Prescription Benefit Card. |

NCCED will be responsible for obtaining NCCED/PBCs with each person's name printed on the card. |

NCCED will mail the personalized NCCED/PBCs in bulk to the CDC or person requesting the card(s) for distribution. |
 |
Each CDC or requestor will be responsible for delivering the PBCs to the individual cardholders, along with a copy of the list of participating pharmacies and contact numbers. |

The CDC should refer all questions regarding prescription medications, pharmacies, mail orders, etc to the toll-free Pharmacy Help line that will be provided. |
